About Dr. Raul Vazquez,
M.D. F.A.A.F.P.

President & CEO

Dr. Raul Vazquez is a Puerto-Rican primary care physician and philanthropist who has dedicated his career to serving underserved and underrepresented communities. Born in the South Bronx, New York to a single mother and raised with two younger brothers, Vazquez was inspired to become a doctor after witnessing the difficulties his uncle faced when seeking medical care in a language that the staff and physicians did not understand. He was the first in his family to graduate from high school, and went on to attend Fordham University for his undergraduate degree and the State University of New York at Buffalo for his Medical Degree.

After completing a family medicine residency program in Buffalo, Vazquez became board certified and a Fellow of the American Academy of Family Physicians. Throughout his career, he has held numerous administrative and staff physician positions and has served as an associate clinical professor at the State University of New York at Buffalo’s Department of Family Medicine. In addition to his clinical work, Vazquez has also served as a speaker for pharmaceutical companies such as Pfizer, GlaxoWellcome, Eli Lilly, and Merck & Co. on the use of medications to treat various medical conditions.

In 1996, Vazquez founded Urban Family Practice P.C., a private family practice located on the Lower West Side of Buffalo. In 2003, the practice moved to a fully computerized facility, and in 2012 it was designated as a Provider-led Medicaid Health Home by the New York State Department of Health. In 2009, Vazquez founded Westside Urgent Care and the Greater Buffalo United Independent Physician Association, later known as the Greater Buffalo United Accountable Healthcare Network (GBUAHN). In 2016, he founded the Greater Buffalo United Accountable Care Organization (GBUACO). Through these companies and his work as an advocate for minority communities, Vazquez has worked to overcome biases and improve healthcare access and outcomes for underserved populations.

In addition to his professional achievements, Vazquez has also been recognized for his philanthropic efforts. Along with his wife Toni, he co-founded the Raul and Toni Vazquez Foundation in 2008 to support non-profit health care organizations and enhance their performance through research, public education, and advocacy. In 2015, the couple was awarded the Buffalo Urban League Family Life Award for their contributions to the community. In 2016, Vazquez was appointed to the Statewide Multicultural Advisory Committee. Through his dedication to healthcare and philanthropy, Vazquez has made a significant impact on the lives of those he serves.
